Project Outreach provides case management services to people with mental illness who are homeless or at imminent risk of homelessness.  It is also referred to as Somerset County PATH, which stands for Projects for Assistance in Transition from Homelessness.  The program works with Somerset County residents and links them to mental health, entitlements, and housing. 

The program staff will customize services to suit the needs of each individual.  A service plan is developed with the consumer to outline desired assistance, which may include linkages to mental health services, physical health services, and community resources, as well as to permanent housing.  The program also assists with applying for entitlements such as temporary housing, Food Stamps, Welfare and SSI/SSDI.  Once immediate mental health needs are stabilized, the program helps people to apply for permanent housing.  Case management services are continued for up to six months to assist the person in maintaining that housing successfully.
